What should a damaged item claim form ask for?
At minimum: order number, the email used for the order, which item is affected, a description, photos of the item, the outer box and the shipping label, and whether the customer wants a refund or a replacement. Carriers usually ask for photos of the packaging too (UPS, for example, asks for photos and may inspect the package), so ask customers to keep the box.
Is the claims tracker template really free?
Yes. Both downloads (Google Sheets CSV and Notion CSV + setup guide) are generated in your browser and download straight away. The email signup is optional and only for update emails.
How do I import the tracker into Google Sheets?
Download the Google Sheets CSV, open Google Sheets, choose File → Import → Upload, and select the file. The Days left column uses a formula that works after import. Delete the example rows when you start.
How do I import the tracker into Notion?
Download the Notion CSV, then in Notion use Import → CSV. Follow the included markdown guide to set property types (Select for Status, Date for dates) and paste the Days left formula.
Do I need Make to use this?
No. Make is optional: it can send each new form submission to your sheet and helpdesk automatically. You can also copy rows by hand.
